Valoración del tratamiento ortopédico
La ortopedia craneal con DOC band® es una técnica nueva que CTI emplea desde el año 1986. Con ella se obtiene una mejoría sustancial de las deformidades posicionales del cráneo del tipo de la plagiocefalia (39-43),la braquicefalia y la escafocefalia, siendo superior al tratamiento convencional conservador (41).

Las molestias derivadas de su uso son mínimas y soportables, siendo el eritema cutáneo en la zona de compresión la más frecuente, y obliga en ocasiones a hacer pequeñas correcciones en la banda. No se han observado complicaciones importantes.

Con la técnica DOC band® se puede llegar a tratar y mejorar pacientes hasta los dos años de edad (44). De todas formas, a partir de los 12 meses de edad, los candidatos deberán ser muy bien escogidos. En los pacientes mayores de dos años el cráneo deja de tener una buena respuesta al remodelado externo debido a su rigidez y la disminución del crecimiento circunferencial.

Los resultados estéticos son generalmente excelentes, con un buen remodelado de la bóveda craneal hundida (principalmente en la región occipitoparietal), un realineación de la parte alta de la cara, siendo el retroceso de la base del cráneo menos satisfactorio, en especial en los niños mayores.

Con la ortopedia craneal se aporta un procedimiento que ofrece buenos resultados a los pacientes afectados por medio de un proceso no agresivo, cómodo y sin recidivas, recuperando el aspecto estético perdido en un plazo relativamente corto de tiempo.

4- Cirugía

Antes de que la plagiocefalia posterior de origen posicional fuese reconocida como una entidad propia, al detectarse su gran aumento a principio de las años 90, algunos cirujanos proponían el tratamiento quirúrgico de estas deformidades, probablemente por asimilación de las plagiocefalias por sinostosis lambdoidea. Sin embargo estas intervenciones exponían a los niños al riesgo de una intervención craneal (en particular el riesgo de hemorragias per y postoperatoria) y además los resultados eran decepcionantes ya que el niño volvía a colocarse en su cuna en la misma posición que antes de la intervención y se reproducía la deformidad (7).

Únicamente en aquellas deformaciones importantes, que consultan a una edad demasiada tardía para beneficiarse del tratamiento ortopédico o cuando éste ha fracasado, la cirugía puede plantearse como el último recurso terapéutico(45).
